Guardian Call Request Form
Please complete the following information if you request to receive welfare check calls from the Ogle County Reverse 9-1-1 system Guardian Call. This form is needed for all requests, regardless of type of telephone service you use. Guardian Call allows the monitoring of the health and well being of senior citizens or other homebound individuals by scheduling automatic "check in calls". Guardian Call sends calls on predefined days and times to individuals to check on them and receive feedback. If the system does not receive back the proper touch-tone response (the "OK Code"), it will automatically notify your provided contact persons. If the subscriber is away from the home for an extended period of time such as on vacation, hospitalization, etc. please notify the Ogle County Sheriff’s Office to suspend the service.
